My Medical Choice

MY MOTHER fought cancer for almost a decade and died at 56. She held out long enough to meet the first of her grandchildren and to hold them in her arms. But my other children will never have the chance to know her and experience how loving and gracious she was.

We often speak of “Mommy’s mommy,” and I find myself trying to explain the illness that took her away from us. They have asked if the same could happen to me. I have always told them not to worry, but the truth is I carry a “faulty” gene, BRCA1, which sharply increases my risk of developing breast cancer and ovarian cancer.

My doctors estimated that I had an 87 percent risk of breast cancer and a 50 percent risk of ovarian cancer, although the risk is different in the case of each woman.

Only a fraction of breast cancers result from an inherited gene mutation. Those with a defect in BRCA1 have a 65 percent risk of getting it, on average.

Once I knew that this was my reality, I decided to be proactive and to minimize the risk as much I could. I made a decision to have a preventive double mastectomy. I started with the breasts, as my risk of breast cancer is higher than my risk of ovarian cancer, and the surgery is more complex.

On April 27, I finished the three months of medical procedures that the mastectomies involved. During that time I have been able to keep this private and to carry on with my work.

But I am writing about it now because I hope that other women can benefit from my experience. Cancer is still a word that strikes fear into people’s hearts, producing a deep sense of powerlessness. But today it is possible to find out through a blood test whether you are highly susceptible to breast and ovarian cancer, and then take action.

My own process began on Feb. 2 with a procedure known as a “nipple delay,” which rules out disease in the breast ducts behind the nipple and draws extra blood flow to the area. This causes some pain and a lot of bruising, but it increases the chance of saving the nipple.

Two weeks later I had the major surgery, where the breast tissue is removed and temporary fillers are put in place. The operation can take eight hours. You wake up with drain tubes and expanders in your breasts. It does feel like a scene out of a science-fiction film. But days after surgery you can be back to a normal life.

Nine weeks later, the final surgery is completed with the reconstruction of the breasts with an implant. There have been many advances in this procedure in the last few years, and the results can be beautiful.

I wanted to write this to tell other women that the decision to have a mastectomy was not easy. But it is one I am very happy that I made. My chances of developing breast cancer have dropped from 87 percent to under 5 percent. I can tell my children that they don’t need to fear they will lose me to breast cancer.

It is reassuring that they see nothing that makes them uncomfortable. They can see my small scars and that’s it. Everything else is just Mommy, the same as she always was. And they know that I love them and will do anything to be with them as long as I can. On a personal note, I do not feel any less of a woman. I feel empowered that I made a strong choice that in no way diminishes my femininity.

I am fortunate to have a partner, Brad Pitt, who is so loving and supportive. So to anyone who has a wife or girlfriend going through this, know that you are a very important part of the transition. Brad was at the Pink Lotus Breast Center, where I was treated, for every minute of the surgeries. We managed to find moments to laugh together. We knew this was the right thing to do for our family and that it would bring us closer. And it has.

For any woman reading this, I hope it helps you to know you have options. I want to encourage every woman, especially if you have a family history of breast or ovarian cancer, to seek out the information and medical experts who can help you through this aspect of your life, and to make your own informed choices.

I acknowledge that there are many wonderful holistic doctors working on alternatives to surgery. My own regimen will be posted in due course on the Web site of the Pink Lotus Breast Center. I hope that this will be helpful to other women.

Breast cancer alone kills some 458,000 people each year, according to the World Health Organization, mainly in low- and middle-income countries. It has got to be a priority to ensure that more women can access gene testing and lifesaving preventive treatment, whatever their means and background, wherever they live. The cost of testing for BRCA1 and BRCA2, at more than $3,000 in the United States, remains an obstacle for many women.

I choose not to keep my story private because there are many women who do not know that they might be living under the shadow of cancer. It is my hope that they, too, will be able to get gene tested, and that if they have a high risk they, too, will know that they have strong options.

Life comes with many challenges. The ones that should not scare us are the ones we can take on and take control of.

    5. 1:46 preventive ang ginawa nya and i salute her. pag ganyang sitwasyon, di ka magpapaopera dahil lang binola bola ka ng doktor. i am a survivor and i tell you hindi madaling magdesisyon. emotionally, physically and financially draining ang mga procedures and treatments pero ang importante buhay ako. thankful na lang at me reconstructive surgery, yun nga lang mahal nga talaga kahit dito sa pinas.

FYI lang ha, hindi naman yan basta basta sinabi na lang ng doctor na "Hoy Angelina magkakaron ka ng breast cancer, tanggalin na lang naten para mas safe ka at para mas yumaman ako." Basa basa din ng mabuti, nagkaron sya ng test, BRCA1 Genetic testing. Dun malalaman kung carrier ka ng cancer or may possibility kang magkaron ng cancer sa genes mo. That test costs millions. Besides, Mayo Clinic ang nagtest sknya. Google google din tungkol sa Mayo Clinic.

      She is so brave to do the mastectomy, and so selfless. Hello, masiraan ka nga lang ngipin, mabunutan or maging pustiso sobrang nakakapanghinayang at masakit na sa dibdib, eh yung mawalan ka pa ng dibdib, sa tingin mo madali yun? She did it kasi sobrang mahal nya yung mga anak nya. Yun lang yun.

    13. Bilib ako sa tapang niya! To actually have the double mastectomy even if walang kasigurohan na magkakaroon siya ng breast cancer, took great courage. Pero mas nakakabilib pa na nilabas niya eto and ni reveal sa buong mundo para makapagbigay ng kaalaman sa mga tao at para makatulong sa ibang mga babaeng nasa similar na sitwasyon. She actually didn't have to do that because she had reconstruction surgery and people would not know that she had a double mastectomy done.

      She did this on her own terms and in her own words. Nakakabilib siya at nakaka inspire.

Napabilib ang reporter when she found out about Angelina's operation because Angelina went to Congo just weeks after the second surgery. Sobrang hirap daw ng trip na yun, malayo ang lugar, bumpy roads, difficult conditions, and the only person in the group who had no complaints daw about anything was Angelina. Sobra siyang napa bilib kay Angelina especially after knowing about the operations.

  7. She is waethy, sha has a choice. The gene test alone costs around $5000. While double mastectomy will cost around $50k then reconstruction will be another cost. Insurance will not pay for gene test.while for the mastectomy, the insurance will only pay if you are in diagnosis stage meaning it is confirmed by doctor that you have tumor.

    While for those who can't afford it, there is no prevention, you can only avail the surgery paid by insurance when you are found to have tumore or cancer.

  9. I salute her, but then I'm sad. Kase, para rin yang castration sa guys. I mean, ok, she carries the breast cancer gene which gives her 87% chance of getting cancer.

    BUT, what if her chance of getting it is in the 13%?

    I know it's really "to each his own," pero I still don't think prophylactic or preventive mastectomy is always the correct thing to do.

    But I think mas courageous ang millions of women who are suffering because of the big C. Angelina is fortunate because she has the money to cover the expenses. Yung gene test pa lang ang mahal na. Ordinary American women do not HAVE options if they are broke. Hindi lahat kasing yaman ng homewrecker na ito

  14. most comments here missed the point why she did what she did. she wanted to stick around longer for her kids! why take a chance of contracting it when you have the means to detect it early enough not to cause any further heartaches in the future. i think what she did was very brave and noble, very unselfish act brought about by her tremendous love for her children.
    i can totally relate to this story, except it's my husband who had to do a radical prostatectomy due to early prostate cancer detection. like her with a history of cancer in the family, my husband was also very vigilant having his PSA level checked bi-yearly. to cut the story short, he and i elected to have the surgery even if the biopsy only showed 1 out of 12 spots had cancer cells. he could not have had the surgery since it's small and dormant but he still did. it's the same thing with angelina why he did it, so he can be around longer, or at least plan on being around longer, for our kids and our grandson. bec of the surgery, he can no longer have a satisfying sex life, it's one of its drawbacks. it's hard for him, but hey, it's a small price to pay if the trade-off is for us to be able to dream of getting old together.
    anyway, as unconventional as it gets with this brangelina household, i've always been a big fan of the couple and what she did only validated why she's one celebrity deserving of my admiration.

  22. I admire her fo this brave act.. Iilan lang ba sa mga babae ang kayang magpa-mastectomy.. Even those with breast cancer when told na kailangan na magpamastectomy eh talagang dumadaan sa depression.. Lalo pa kaya si Angelina she only has 87% chance to acquire this cancer but she still took the risk.. She must be going through a lot of hardship when her mother was diagnosed and died of this Cancer.. She doesnt want her kids to go through the same torment she experienced when her mother acquired the disease.. She is working hard to earn money, san pa ba niya gagamitin yung pera kung hindi para sa kanya at sa family niya.. Anyway, to Angelina whether it's real boobies or fillers, it doesn't make you less of a woman.. What is important is you did your job as a mom to stay healthy so you can be with your kids in the future..

  25. Just a thought. What if malaman ulit niya na at risk siya sa lung cancer? Liver cancer? Kidney cancer? Colon cancer? Ipapatanggal rin niya lahat? Baka ang pagkaubos ng organs niya pa ang ikamatay niya at hindi cancer. I read last night na ipapatanggal naman niya ovaries next. Medyo drastic in my opinion. Kahit anong pagprevent mo kasi sa isang bagay kung yun ang nakalaan para sayo mangyayari at mangyayari yun.

    1. you can live without your breasts/ovaries...if there is a cancerous tumor (or the beginnings of it) in those areas, then at least it's prevented early..there's nothing wrong with these surgeries, she's high risk to begin with anyway...but then again, there's only so much you can do..if you're meant to have the disease, then you're meant to have it (regardless of all the preventative tx you've undergone)..but let's not wish that for anyone....

For me who lost an aunt to breast cancer, another aunt who has breast tumor now but afraid to be diagnosed (she is 70), and a mom who have undergone total hysterectomy because of ovarian tumor (it was benign, thank God), i think what Angelina did and told us is enlightening. There is no need to live the rest of our lives in fear. Since not everyone is as wealthy as her to afford that kind of test, regular check up will do for those in high risk like me.

Most of the commenters here failed to do more reading about this, di nio ba nabasa ung sinabi nia na it's better to have done the operation kasi that gives her children a feeling of security that she will be living more years with them than explaining to them why she won't. It's a mother kind of thing and only a mother can understand her and her reasoning (whereas nalang na open minded ka)

    Reconstruction happens afterwards. Some women, like Angelina, get implants; most of the times, these days, they're saline implants. Other women take fat from their hips or fat from their bellies and get breasts made out of that.

Regardless of her bashers' opinion, for me she is a great mom for thinking she needs to live long so she can take care of her kids! i can see her children as the center of her world, and not herself. she is indeed a great woman inspite of her imperfections and definitely a great mom! This should be what celebrities be like. They don't have to be perfect, they just have to be real.
